Both patients showed consciousness disturbance, complete ophthalmoplegia and areflexia of bilateral lower extremities. The characteristic neurological findings as well as MR findings such as lesions at dorsal medial nuclei of the thalamus and periaqueductal regions enabled early diagnosis of the disease. Because thiamine is required in glucose metabolism as a coenzyme, carbohydrate loading in the presence of marginal thiamine storage can result in the development of Wernicke's encephalopathy. It should be noted that Wernicke's encephalopathy can easely be induced by prolonged intravenous glucose infusion without vitamines.
