In 1971, Japan established an unique health insurance system : so-called obligatory mutual help medical insurance, and has been able to receive a relatively high level of medical care through it. Under this medical system, university hospitals have been able to have a honey-moon for about 20 years. However, because of the enormous increase of medical costs for the elderly, the relationship between the two is drastically changing. In the past decade, most university hospitals had to contend with the problem of deflated medical costs due to health insurance. Recently, it has become obvious that there is a clear gap in a view height between the two. University hospitals have responsibility to develop advance medicine and /or in innovation in a special field, but the insurance system is only directed at how to deliver standard medical care to all people. Year by year, the gap widens.
