In the present study, localized brain proton MR spectra and T_2* weighted MR images were acquired in the occipital visual cortex under photic stimulation (PS) from normal volunteers, and patients with migraine and mitochondrial encephalomyopathies (Kearns-Sayre syndrome) (KSS) using whole-body 1.5T clinical MR system. In the controls, lactate elevation was detected after the onset of PS. With continued PS, there was a decrease from the maximum. After PS was stopped, the lactate decreased almost to the baseline value. These results are consistent with the view that PS activated brain visual cortex, which stimulated glycolysis more predominantly than oxidative pathway. In the patients with migraine and KSS, the resting values of occipital lobe cortex lactate were significantly higher than those of controls. Furthermore, lactate values in KSS were significantly higher than those of migraine. However PS did not increase lactate significantly suggesting the blunted glycolysis in the impaired oxidative metabolism. The T_2* weighted MR images of the occipital visual cortex of human brain were studied by means of fast low-angle shot (FLASH). Among normal volunteers, elevation of image intensity up to 2% was observed in primary and associative visual cortex. After the PS was turned off, the MR signal fell below the pre-PS baseline level.
