Several prospective studies in western countries, especially in the USA including the Framing-ham study, have shown a positive linear relationship of weight to ischemic heart disease. However, many epidemiological studies have not shown this linearity. This inconsistency makes the significance of obesity or overweight as a coronary risk factor uncertain. Thus, we also studied the consequence of overweight as a risk factor for coronary atherosclerosis by a case-control study performed in Niigata prefecture in Japan. The cases consisted of 144 men with significant coronary artery stenosis aged 60 years or younger. Two age-matched men were chosen for every case as the controls. Mean value of body mass index (BMI) was significantly higher in the cases (24.0±2.7 kg/m^2) than in the controls (23.0±2.7 kg/m^2, p<0.01). However, the proportion of overweight men (BMI≧25.7 kg/m^2) was similar in the cases (21.6%) and in the controls (15.5%). The odds ratio of overweight for coronary atherosclerosis (1.20, 95% confidence interval: 0.58~2.49), estimated by a multiple logistic regression analysis, was also not significant.
