Importance of blood pressure control is also recognized in prevention of retinopathy and nephropathy, although it has been well recognized in ischemic heart disease. We studied the mechanism of endothelial cell damage in diabetic patients using soluble thrombomodulin (sTM) concentration as endothelial cell marker. No correlation was found between sTM and HbA1c in diabetic patients without any complications, although high sTM concentrations were found in patients with HbA1c of more than 6 %. Short-term glycemic control could not reduce sTM concentration in the diabetic patients. Oral administration of aldose reductase inhibitor, Eparlestat, for four weeks could not reduce sTM in diabetic patients.
