Fifty percent of the arteries were patent (TIMI prade 2-3) in patients who received intravenous t-PA (N), and only 26.6 percent were patent in patients who did not receive IV. After intracoronary t-PA (IC), patency rate increased to 52.6 percent in patients without IV. In patients treated with IV follwed by IC, TIMI grade 2-3 flow was achieved in 76.9 percent of the infarcted-related arteries. PTCA achieved TIMI grade 3 flow in all 22 patients, including 15 patients in whom IV+IC failed to achieve TIMI grade 3 flow. Coronary artery patency rate was the highest in the IV+IC group as compared with the IV or IC group.
