 note = {According to the statics, increase of ratio of old age population and reduction of family scale has also been problematic here in Niigata city. Therefore, it is essential to construct coordinated home delivery health care system for severely impaired elderly patients immediately. Now in Niigata city, the clients can use certain medical or social welfare services, such as treatment at home by doctors, home nursing, home help service by Niigata City Welfere Corporation, and other services at fascilities for social welfare for the aged. Although integrated social care system for intractable neurologic patients has been promoted since 1991, when“Niigata City Case Conference for Intractable Disease”was established, that for elderly impaired patients is not sufficient yet. To offer integrated and continuated home care services for elderly persons, effective regional case conference should be organized by the members from home nursing station, home care back-up center, regional health center and medical association.},
