The problem of speech has been greatly reduced by repairing the palate with push back method. However, midfacial growth retardation still remains as an onerous problem. To overcome this problem, two-stage operating method combination with the use of Hotz plate as a presurgical orthopaedic guidance has been proposed by Hotz and Perko. Recently we managed the cleft lip and palate patients by this method. In this method, ultimately we have observed excellent maxillary growth, normal arch and almost normal occlusion, which have not observed at one stage closure using the push back method.
