We encountered a 64-year-old woman who had episodic chest pain and diagnosed her illness as MVO with apical aneurysm. Since significant coronary stenosis was not found by coronary angiography, intracoronary acetylcholine infusion was tried to investigate the cause of symptom. Coronary spasm was induced with ST change and chest pain similar to usual attack. This imformation was useful for planning medical therapy. Few HCM cases complicated by vasospastic angina pectoris (VSA) were reported. We found no report of MVO patient accompanied with VSA.
