The rupture of plaques is now considered to be the common pathophysiological substrate of the acute coronary syndromes that include unstable angina, acute myocardial infarction and sudden ischemic death. To examine whether or not the levels of NOx (nitrite and nitrate) and thrombomodulin (TM) in coronary circulating blood reflect endothelial dysfunction in coronary atherosclerosis, the levels in plasma obtained from ostium of left coronary artery and coronary sinus of patients who complained of chest pain were evaluated in relation to their coronary angiographic findings. Our study revealed a decreased NOx level and an increased TM level in coronary circulation in patients with severe coronary atherosclerosis implying severe coronary endothelial dysfunction.
