The relationships among IC deposition, inflammatory cell infiltrate, E-selectin expression in endothelial cells and mast cell degranulation in the disease were analyzed with confocal laser scanning microscopy. IgA is always deposited mainly in capillary loop, but in most of the lesions examined only a small amount of inflammatory cell infiltrate is observed in the blood vessels. E-selectin was expressed in endothelial cells of blood vessels in fifteen of thirty-two patients (46.9%). Most blood vessels with E-selectin-positive endothelial cells were the horizontal plexus, and dense inflammatory cell infiltrate was always observed around the blood vessels. In lesions from seven patients the number of avidin-binding cells around blood vessels was inversely proportional to the density of inflammatory cell infiltrate. In historogically uninvolved site from four of them, the number of avidine-binding cells was significantly decreased compared with normal skin. However, there was no significant difference in the number of c-kit positive cells between involved and uninvolved vessels from all of the four patients. In addition, fluorescence intensity of avidin-binding granules was much decreased in involved vessels compared with uninvolved one. In capillary loop in skin lesions from nine patients, there was no difference in the number of avidin-binding cells between lgA positive and negative vessels. Fluorescene intensity of avidin-binding cells was markedly decreased around the E-selection-expressed vessels.
