SAはHPから発生し, 遺伝子不安定性(MSI)を示す大腸癌の前駆病変と考えられているが, その組織発生の詳細については明らかにされていない. 本研究では, 外科切除および内視鏡的に摘除された219例のSAの組織構成成分を病理組織学的に解析し, SAの組織発生について検討した. その結果, SA単独例が37.0% (81/219), SAとHPの併存例(SA+HP)が22.4% (49/219), SAと異型過形成性ポリープ(atypical hyperplastic polyp) (AHP)の併存例 (SA+AHP)が15.10%(33/219), SAとADの併存例 (SA+AD)が25.6% (56/219)であった. SA+HP, SA+AHPのそれぞれ59.2%と54.5%で, SAはHPまたはAHPに包含されており, SAはこれらの病変を母地として発生したと考えられた. SA+ADの80.3%はSAとADとがモザイク状に混在しており, これらの病変はSAとADの両方向への形態形成を示す病変として発生したと推定された. SAのみからなる病変は他の病変に比べ大きさが小さく, 前駆病変を介さず正常粘膜から直接発生したと考えられた.
