Dysplasiaは軽(low-grade dysplasia: LGD) と高度 (high-grade dysplasia: HGD) に大別されるが, そのいずれが, 食道扇平上皮の発癌早期段階かは明らかではない. このことを明らかにするため, 本研究では, 食道扁平上皮癌で高頻度にみられるp53遺伝子異常に着目し, 扁平上皮癌とそれに併存する上皮内腫瘍 (LGD, HGD, CIS) のp53遺伝子変異および同蛋白過剰発現を検討した. 内視鏡的摘除ホルマリン固定食道扇平上皮癌10例 (粘膜固有層浸潤癌: M癌4例, 粘膜下層浸潤癌: SM癌6例) を対象とし, LGD, HGD, CIS, M癌部, SM癌群別にp53免疫染色を評価し, それぞれの領域からマイクロダイセクションによりDNAを抽出し, p53遺伝子のエクソン5～8をPCRで増幅し, シークエンス解析により同遺伝子変異を検索した. p53遺伝子変異はLGDの83.3%, HGDの50.0%, CISの71.4%, 浸潤癌部の50.0-75.0%に認められた. LGDとHGDでは, 1例を除き, 同一病変内に併存するCISおよび浸潤癌部と変異コドンが共通するか, もしくは両者とも wild-type であった. これらのことから,p53遺伝子変異の観点からみた限りは, LGDおよびHGDは同一病変内のCISや浸潤癌部と組織発生学的連続性があることが推定された. 同一病変内のCISと浸潤癌部では遺伝子変異は単一の変異コドンに生じていたが, LGDとHGDでは変異コドンに多様性 (複数の変異コドンが存在) がみられた.
