In acne vulgaris, abnormal follicular keratinization is important for comedo formation, yet no precise mechanism of comedogenesis is known. We investigated the relationship between water-barrier function (WBF) and lipid composition of stratum corneum in 40 male patients with facial acne and 27 control subjects. WBF which was evaluated by transepidermal water loss was significantly lower in acne patients. No significant differences between acne patients and control subjects were noted in all lipid fractions except sphingolipids (ceramides and free sphingosine). These results suggest that diminished WBF caused by decreased sphingolipids may be responsible for comedo formation, because barrier dysfunction is accompanied by abnormal follicular hyperkeratosis.
