A 55-year-old male was hospitalized for examination of an abnormal shadow in the right upper lung field on chest roentgenogram. Plain chest roentgenogram showed a patchy area with high density in the right upper lung field, and vascular shadows connected to the hilum. In addition, pulmonary arteriogram revealed a lack of blood supply to the right segment 2; the right segment appeared to be normal fiberoptic bronchoscopy. Aortogram demonstrated a systemic arterial supply to the right segment 2 region by anomalous vessels stemming from the intercostal and subclavian arteries, with a drainage of blood into the pulmonary vein. These findings led to the diagnosis of a systemic artery-pulmonary vein fistula. The anormalous condition is considered to a variant type of pulmonary arteriovenous fistula.
