The management of pregnant women complicated with autoimmune disease is one of the most important issues for obstetricians. It is well-known that the reproductive performance is tend to be poor in patients with autoimmune disease, especially in patients with systemic lupus erythematosus. In this context we investigated the outcome of pregnancy in patients with autoimmune disease, mainly in patients with SLE. Out of 29 pregnant women with SLE allowed to continue their pregnancy, in 6 cases pregnancy had resulted in stillbirth, and in 2 it had resulted in early spontaneous abortion, thus in 27.6% of them the pregnancy had resulted in fetal miscarriage. In prenatal course, 28.8% of patients with SLE had showed deterioration of the disease, and 9.6% of them had showed after the termination of their pregnancy. Out of 27 babies delivered by mother with SLE, only 12 babies (44.4%) were born as appropriate-for-date-baby, and 7 babies (26.0%) were dead either before or after delivery. These data clearly demonstrated the necessity of careful observation and management of the pregnant women with SLE. In addition to these investigations, we examined the autoimmune aspects of patients with recurrent fetal wastage. Out of 62 patients, in 11 cases the antinuclear antibodies (ANA) were observed in their serum, and especially secondary recurrent aborters revealed high incidence of positive ANA, suggesting the existence of subclinical autoimmune disease in patients with recurrent fetal wastage.
