In order to monitor spinal cord function during spine surgery, we recorded the conducted spinal cord potentials (C-SCPs) from the epidural space in 134 patients with scoliosis who underwent correction surgeries. The conducted SCPs consisted basically of three spike-like components followed by slow components as we previously reported. The potentials, in whom the decreases in amplitude and prolongations in latency of the SCPs were within 50% and 0.2 ms, respectively, during surgery, developed no significant neurological deteriorations postoperatively. Two patients in whom the SCPs disappeared without recovery during surgery suffered from severe neurological deficits. There were no significant differential effects of spine tractions on each of the three spike-like potentials. Slow waves were similarly affected by traction forces on the spine as the preceding spike-like potentials. Thus, we proposed that manipulation of the spine must be suspended when the latencies of the conducted SCPs are prolonged for more than 0.2 ms or their amplitudes are decreased to less than 50% of the preoperative values.
