Fractures of the temporomandibular joint are not so rare that may be encountered in many cases of facial trauma. Therapy for these fractures is divided into two concepts, sanguinal reduction-fixation and conservative therapies. At the clinical stand point, it is not satisfactory only to reduce the fractured piece to the anatomical relationships. There may be very persistent cases which would possibly leave functional deterioration postoperatively, i.e, limitation of mouth opening, open bite, pain with jaw movement, clicking on mouth opening, so forth. It is well advocated that regaining of temporornandibular functions is paramount to take treatment for these fractures. In order to evolve the prognostic satisfaction, precise preoperative evaluation of the fracture, adequate selection of the modality of therapy, and rehabilitation of the temporomandibular function immediate after release of fixation are inevitable. The anatomical location of the fracture in the condylar process, inclination of the fracture line, dislocation of the fractured condyle, bilateral fracture or unilateral, deviation of the mandible, and concomitant fractures should be involved in the preoperative evaluation. Rigid fixation is’expected to reduce the duration of intermaxillary fixation. Intermaxillary elastics with or without pivot on the posterior molar would be superior to the conventional intermaxillary fixation to obtain better results.
